After one year and nine months of the pandemic, Nathália Fraporti is still afraid. Her routine as an intensive care physician in the ICUs of the four hospitals in São Paulo where she took care of covid-19 cases shows how powerless a professional can be. Without drugs to fight the disease, the way was to alleviate the symptoms and wait. The patient had to fight the virus on his own. She's lost a lot of patients like that. When Brazil started vaccinating the population in early 2021, it was hopeful that this nightmare would come to an end.
Although slow, the country's response to vaccination is generally positive. By January 4, 2022, more than 143.7 million people had completed the vaccination schedule, a number that represents 67% of the country's population, and more than 26 million had taken the booster dose. The increase in vaccination across Brazil has reduced the number of deaths from the disease and has caused many ICU beds dedicated to covid-19 to be closed.
Fraporti thought she might envision a somewhat more relaxed routine in hospitals – until she began treating severe cases of women infected with the coronavirus who had been banned from being vaccinated by their husbands. The stories seemed like a flashback to the time when she was in gynecological residency in Taubaté, in the interior of São Paulo, and attended to women with sexually transmitted infections who were prevented from treating themselves by their partners.
The doctor detailed for Intercept this harrowing routine at the hospital, the result of a mixture of machismo and misinformation. Her account was edited for clarity, and so we don't expose her patients, we use fictitious names.
Renata was 65, if I'm not mistaken. She was the most serious patient we had in the hospital. She was hospitalized in early October, along with her husband, Fernando. He recovered, but she stayed. As the covid-19 cases had dropped so much, there was a time when she was the only patient with the disease in the ICU. I called the family almost every day and talked to her sisters. I remember when she arrived at the hospital, I asked her why she hadn't been vaccinated. In October, Renata would already be close to taking up to the third dose of the vaccine. She said she wanted to, but her husband forbade her. She kept wanting the shots.

When a patient has severe acute respiratory syndrome and spends a lot of time lying down, as in Renata's case, he finds it difficult to send oxygen from the lung to the blood. In this case, we can prone the patient, which is a maneuver in which we turn him on his stomach. It's as if we took off this weight that is on top. Thus, the lung is able to oxygenate itself more easily.
We need at least six professionals to perform the maneuver without taking the breathing tube out of place or causing any change in the patient's access. Renata had a lot of benefit every time we did the maneuver. Despite their advanced age, there were 13 prones. This is quite a thing, she was one of the patients we were most prone to in the hospital. Until the moment she stopped responding to the procedure. This is because, when the breathing tube stays in the airway for a long time, it ends up sticking to the mucosa. We had a tracheostomy, a surgery in which we cut the patient's throat and insert a small tube into the trachea to make breathing easier. But Renata had already spent more than 20 days intubated. It reached a point where she no longer had covid, we were only treating the sequelae of the disease, such as pulmonary fibrosis.
After a month and a half in hospital, she died. I wanted to understand what had led Fernando to do what he had done, not to get vaccinated, nor to allow his wife to do so. He told me that he had never had a vaccine in his life. With all these fake news circulating, he came to believe that the vaccine was bad for his health. He even told me that the virus had been manipulated, created in a laboratory. With an air of biological warfare, you know? He believed that. And he thought the vaccine was part of the same manipulation. He thought he was protecting himself and his wife.
In the calls to her sisters, when she was still hospitalized, I knew that Fernando was very sorry. He was saying all the time that he was going to lose his wife, because he wouldn't let her get vaccinated. He became immunized when he left the hospital, but feels guilty for depriving his wife of this opportunity.
I followed two other patients, Julia and Rose, who were also not vaccinated because of their husbands. Julia was around 35 years old, and Rose, in her mid-40s. The comrades were staunch supporters of the Bolsonaro government and believed everything the president said. They ended up having the same idea, because their husbands are the people they talk to the most, with whom they form an opinion. In these two cases, their husbands didn't allow them to get vaccinated, but they were also influenced to some extent.
Julia and Rose were admitted to the ICU, but did not need to be intubated. They recovered and then received the vaccine. I talked a lot with them. Both were sorry - like their husbands.

When we are in the hospital, we observe a very fragile moment in the family. You have to be very sensitive to deal with this. It is not a time to judge, to point out what these people have decided, but they themselves bring it to us: "look, I regret it, because I can see the risk I am taking, the problem I created and could have avoided if you would vaccinate me”. Very sad that people only realize it at these times. Just when death may be inevitable.
These husbands' impositions on women also came up a lot when I was doing gynecological residency. During prenatal care, some patients found they were infected with STIs. As doctors, we advise the patient to tell her husband, because he also needs to undergo exams and, if necessary, treatment. In the case of syphilis, for example, both of them need to be treated, because they will probably continue to have sex and, without the combined treatment, the patient will heal and then become reinfected.
Two things happened a lot: most were embarrassed to tell their husbands, often because they didn't know how they got the disease, whether it was before or during the relationship, and they were afraid of creating friction with their partner. But there were cases in which they arrived at the office already knowing the diagnosis. The husband already had an STI, he was undergoing treatment, she knew that, but he said that she did not need treatment. "This must be nothing, and life goes on!" This is very serious. They are, or should be, informed that this is not the case.
I have already followed cases of babies who died because of this. A patient of mine, Silvana, was married to a husband who had syphilis. During prenatal care, her exams were negative for the disease, but on the ultrasound she could see that the fetus had some changes in the body. When we measured VDRL, a diagnostic test for syphilis, the rates were very high. Syphilis has it. When rates are too high, it can give a false negative. She told us that she knew her husband had the disease, but that he had been treated and that he thought nothing was going to happen to her. Silvana's baby barely opened its eyes and died.
Another patient I followed, Lourdes, was prohibited from treating HIV by her husband. He was infected, undergoing treatment, but he would not let her go to the infectologist. Lourdes already imagined that she was infected, because she had sex with her husband. When she was diagnosed during prenatal care, she was not surprised.
Unfortunately, this is quite common. At the time of residency, I attended to many patients who discovered they had HIV during pregnancy and did not want to tell their husbands. When they told, either they already knew why they had been the carriers of the virus, or they created a fuss saying it was their fault. The vast majority did not like to count.
This is all very sad and revolting. Women need to convince themselves that they have the personal power to do what they want, even if they are not prepared to get out of an abusive relationship - because I understand that a person who prevents you from taking care of your own health in the way that you thinks better is treating you abusively. If these women are not ready yet or have no way out of this relationship, and as long as they are not in private prison, we have thousands of ways to take care of their health.
I've already worked in a clinic where the community worker knocked on the door of his house, taking the vaccine. I've done prenatal care for patients in hiding several times, because women didn't want people to know they were pregnant. They took the vaccine and tore up the receipt afterwards, so no one could find it.. It's not ideal, but at least you're protected, you're immunized. Think of ways to be who you want to be in a way that doesn't harm you.

This wave of disinformation worries me a lot. Since I graduated from college four years ago, I have the impression that people no longer believe the information we give them. My mother is like that. She prefers to believe in WhatsApp, what the neighbor or so-and-so says than in me. We have some fights about it, but she listens to me. She even called me the other day to tell me not to take the third dose of the vaccine, can you believe it? She said: "daughter, there are so many people dying, heart attacks, people who show up with cancer and die a week later, isn't that about the vaccine?" No! It has nothing to do with the vaccine. We've always been vaccinated with a lot of stuff that nobody even knows about and now we're going to blame this vaccine because it's new, because the media kept talking a lot, because the president kept talking? “True, daughter, I think we really have to take care of the emotional one”, she said. So she changed the subject.
My father, on the other hand, is a confirmed bolsonarista. He is from the countryside of Rio Grande do Sul. I always say that he doesn't care about political ideology. He doesn't defend Bolsonaro for the projects that the president has for Brazil, he defends because he has an aligned speech. That thing of “a good bandit is a dead bandit”, “we have to provide weapons to the population to defend itself”. But he got the vaccine, because he got covid-19 before getting vaccinated. He didn't need to be hospitalized, but it was pretty bad. At first, he might even think it was just a “little flu”, as the president used to say, but when he caught the disease, he was quite scared.
My parents are simple people, they own a cafeteria in the interior of São Paulo. They don't have much education. Sometimes I think that maybe they have difficulty giving credibility to this more complex information. I do not know.
In the past, people saw doctors as someone who was there to help. Today, they see us as mercenaries. The news puts a lot more emphasis on medical errors, for people who have been mistreated.
We talk so much – “go get vaccinated, the vaccine is no problem” – we do an educational campaign, clear up so many doubts, and people still choose to follow what they hear at random, without a source, without any confirmation. If this happens to people in our family, let alone people we don't even know who come looking for us to talk, to clear up doubts. I feel so helpless. It seems that nothing we study, what we research to inform people is valid.
It's not enough for us, health professionals, to give lectures at health centers. The population still does not have access to information in the way we would like. We need more tangible campaigns, thinking about how to put it in a soap opera, a film, speak the population's language… Things go viral on WhatsApp because people can understand what is written there.
I honestly feel scared. I'm scared of it never going away, and we always live in this cycle of increasing cases, decreasing cases; we have to mobilize all efforts to reopen the covid beds. We had so much trouble in the pandemic: sedation ended, material ended, there was no ventilation devices, no professionals. So many people were dying that we had nothing to do. We intubated, pronated and waited. I'm really afraid that a new variant will appear, and the vaccine won't catch on.
This is an alert, you know? People need to get vaccinated more than ever, or it will never end. There is no reason to fear the vaccine. Brazil is a country that vaccinates a lot, and very well, for decades. We have a very strong vaccination culture. We have the SUS, which eradicated several diseases reaching a rate of 90% immunized. Unfortunately, this is not the case in all countries. In Europe, there is plenty of vaccine, but many people do not want to be vaccinated. But if they don't get vaccinated there, the virus will continue to proliferate and mutate. I believe in science, but I'm scared of those people who don't get vaccinated.
Today, we only have 10 covid beds in the hospital where I work, but we already have 36. Of the people who are currently in the ICU, most have not had the vaccine. And now we also see cases of elderly people who took the two doses but didn't take the third. I saw a patient the other day, José, 75 years old, who took both doses throughout the year and did not return for the third dose. He recently went on a 15th birthday party, drank from the same glass as the teenagers and ended up catching the disease. Now he is intubated. “Oh, I wasn't even going to take the third one, but now I'm going to vaccinate when I leave here”, he told me. This is a case, but the profile that usually goes to the ICU is unvaccinated. That says a lot. Vaccines work.

Unfortunately Vivekananda, all the info you posted is simply hearsay and based on anecdotal evidence. Which is fine for stories but doesn't amount to scientific proof of something so if it did, I can post just as many stories of people dying in US hospitals because there weren't allowed to be given life saving medicines like Ivermectin, Monoclonal Antibodies and others.....or maybe I should post (again) why India, a country with 4x's the population of the USA has only about half the number of Covid fatalities? (not to mention most of Africa which has a very small vaccination rates and equally small Covid fatality rates)??

Also, when you are taken to court because you ask for a medicine for something like Ivermectin (which is considered VERY safe after 2 billion does have been administered) for your dying with Covid relative and they refuse to give it, why the heck SHOULD people believe doctor's? (and NO, this isn't about the doctor being sued if anything goes wrong because they have already been given blanket immunity here in the USA with regard to Covid treatments).

As I've been saying all along, you should be FREE to get, OR NOT GET the vaccination/boosters but forcing/mandating them is abusive, insane, criminal and a crime against humanity. (Given their experimental nature, untested long term future side-effects, unnecessary risk based on Covid fatalities regarding certain younger age groups, other viable medical options [Ivermectin, monoclonal, etc.] and potential immediate side-effects right after getting the shots [heart issues, permanently disabled, death, etc.).

My prime minister (and a few other VIPs) is by law EXEMPT from ever being forced to take the vaccine.
If he needs to be protected from taking the vaccine, that tells me that the vaccine is more dangerous than the stinking virus.
This aside from the fact that I believe this virus was created deliberately for this great reset and the people who created it will not take this vaccine themselves, nor will they risk creating a virus that will wipe them out as well!
