*Requirements:
PHP - v5.x
MySQL - v5.x
ionCube Loader 4.0.8+ [not needed]
mod_rewrite and allow_url_fopen must be enabled

New upload is coming, just double checking it all, cleaning up test accounts etc and packaging.

Updated links + Virustotal in first post with fixed package + cleaned DB dump.
Delete ALL old files from server and upload new ones. From what I can see there are no errors and things are working as they should be.

Here is the CPA Plugin.
Basically you put the cpa folder in the plugins directory then read the Readme.txt on how to set it up with your CPALEAD account.

For me works fine, and I have no errors in sfreceive.php as bulqr4eto. As I can see the file upgrade.php is missed. For the rest works fine
