For me " hands free management" is I set up the network once and I don't have bother about its operation for weeks or even 1 month. The extra money I mentioned was only if you don't want to touch it for months together, like 3/4/5 months.
However, I anyway login to my network for link building campaigns every week, so spending some 1-2 hr/month to manage the network is not a big deal for me, which is why I don't even spend that $5.
I assume all of us would anyway login to our network at least ONCE in 1-2 WEEKS for link building campaigns, so spending that little extra money is in fact unnecessary. I just mentioned there so that people know they have that luxury.
If you have a VA, then you can in fact handover the entire guide to prepare, setup and operate the network, of course with some of your inputs.
For 2nd point, if you want to scale to 150-200 blogs, then some little extra investment is required as mentioned.

Overall I was very impressed with the guide.
