What makes you think it doesn't work? I've just installed it and it works brilliantly.

I'd also like to know the answer to the 2nd question. I'm not sure if this WP plugin can create a squeeze page like that though...or can it???
the plugin works well, even if there are some bugs (for example it doesn't work correctly for me with not integrated autoresponders).

Squeeze page templates (the OTO) are in the zip file of 1.04 version previously shared. You can use them and put the optlinks code (look for "START OPTINLINK SHORTCODE HERE")
I've found if you try to use the double optin, the plugin fails to work correctly. It's supposed to wait until the person confirms their details before giving them access to the protected page, but I've found it doesn't wait and simpy takes them to the page without them confirming their email.

I've also noticed that people can enter me@me.com, someone@gmail.com etc, etc, which are obviously bogus emails, but the plugin still treats them as a genuine email address and accepts them. I thought the plugin was supposed to check for authenticity to avoid people entering made up email addresses?

In a nutshell, me thinks it aint as clever as the vendors make out...Oh well :(
Yeah I agree, I mean I've sorted my web hosting issues now, but the plugin just isn't as spectacular as I thought. I was having a look around and OptinTango looks very sexy, the elitists are putting it in the VIP section though, so if anyone can get it and share it here, please do.

From my test the plugin (version 1.06 in post 67) doesnt work correctly with not integrated autoresponders as mine.
I tested mailchimp (the free version) and it works well.

Yes, the programmer probably hard coded the folder name in the plugin. Deactivate the plugin, now rename the folder you installed to as "opt" all undercase and without the quotes of course. Now reactivate and see if it's working.

That fixed it for me..
