>> I will setup your email server with these protocols (DKIM, SPF, Sender-ID, and Domain Keys) and that they are properly implemented, the main elements which cause emails to be rejected by recipient mail servers.

Also there are others precautions you need to take like avoid spam traps, avoid large attachments, use spam checkers before sending your emails, avoid spam trigger words and phishing phrases and so on. I can recommend some good reading.

But in terms of server side configuration, this settings will ensure your email will deliver to the recipient inbox.

>> The price is one off. If you sign up with plan 2 or plan 3 then yes you can use it forever. This plan includes the pmta installation files and guide to configure it in case you need to change the server in future.

>> my recommendation is to use pmta as a smtp server. And you can use any sort of email marketing software to send mass email. just use the pmta smtp login details. In this way your email marketing server or domain name wont get blacklisted easily.
