After seeing many requests here, I uploaded it here from some other well known source (Not mine), although it was password protected and optionally asked for survey to get password. You'll able to download the file but that rar file will have password on it. The password is able to download on given link within notepad inside that rar.

This is typical CPA type survey which is outdated for us in here.

I just hope someone able to break this password without us having to fill in that shit survey.
