because you uploading all the zipfile which is wrong. unzip the file, zip only the folder picotheme12 then upload. this should solve all your troubles.

i have test this one after installation some files are missing like menue, wedgets , please upload a full package thanks test again

did you enable the menu's and widgets?

'functions/theme-service.php',

These files are missing from the packages that why its showing error ..
