Kingpin it was good of you to track that down, but that script is V3.1 which dates from 2010.
Don't waste time trying to figure out how to use it. It may not even be compatible with Wordpress 4. Besides, there are much better solutions out there now...

I am actually wanting a good review plugin like the example in the OP any reccomended
