Thanks for the share. I have not tried the plugin yet. But I was just looking as something for doing surveys, and here you are with this plugin.
People reading Ryan Levesque, Ask Formula stuff, then this plugin sounds like it could be just the thing to be able to build the surveys on your website vs using survey monkey or Google Forms....
