Regarding the "termsofuse.php" the one in this share is identical to the one that came with the one I purchased and does not appear to be infected at all. But like suggested earlier. It's only a terms of use page so change it to whatever you like.

No license required unless you want to have documentation should you be asked for it from someone scouting to see who is using this script. Highly unlikely considering how many people are using it.
