The only thing i know thats close to it but it's only for Facebook called "Social Lead Chief".

I came up with an alternative, for those who are still interested in something like this...
Get the "Thrive Content Builder" here --this plugin will allow you to make fully customizable pages
...sales pages, lead capture pages, etc.
They got ready to go templates or you can start from scratch.
All you gotta do is setup your own 'survey' and viola! you got yourself something way better than presell crusher IMO
To even make it better, use the
'Page Transition' plugin to add a cool effect to it.
