About the problem of fatal error, I figured out that it depends on the version of the PHP you are using. Some plugins works well in PHP5 but shows error in PHP 4. So, if you are getting this error it is advisable that you check the version of PHP you are using currently. This could be one of the reason and switching from PHP 4 to PHP 5 really worked in favor of me.

and what about the double dir: eg.:__MACOSX

Use php 5.5 and above versions and what do you want to ask about "double dir: eg.:__MACOSX".

with in the zip file is the __MACOSX directory

this has link to thumbs.png in vc-extensions-cqbundle.php - line 24, remove that and remove thumbs.png on img directory. not sure if this will work either.
uploader this is not a good upload. probably from getnulled sites. please upload a clean one.
