In the pdf they talk about a Super Adsense theme supplied in your Wordpress Adsense system package does anyone have this theme or what will work I did a search and could not find anything
Also is there any other extras goes with this?

Its free theme just search on wordpress and also you can use various theme. Its just simple put your Adsense code on those themes then you go .....
I have that Super Adsense theme, and bought this when it first came out about 5 years ago. To my knowledge it has not been updated and was designed for Wordpress 2.X
Bottom line, you are better off getting one of the many newer themes designed for Adsense
