My "SHORTCODES" icon/button in visual edit mode is GONE?!?!? It's WP 3.9, any thoughts out there?

Here is the newest version as at 6 May 2014. Directly downloaded from Themeforrest, clean and untouched- includes setup guide, child theme and demo data. Enjoy.

Another new version as at 26/5/2014- this is the installable wordpress files only, demo data, user guide etc are in the post above.
